In screen section add
Option "metamodes" "CRT-0: 1280x1024_75 +0+0, CRT-1: 1280x1024_75 +1280+0"
to allow mplayer to go full screen on a single monitor.
Oh yeah, and
~/.mplayer/config has
xineramascreen = 2
